I agree that I am requesting a one-time donation in the form of one [or a set if applicable] of handmade stained glass art created by Zephyr Lane Stained Glass. This item is to be used for the purpose disclosed in the provided information and shall not be used for any other reason. If the event that this item is donated for is cancelled I agree that the item will be returned within 7 days in the condition and packaging that it was sent in. Any damage to the item once it has left the artist’ possession is the responsibility of the event coordinator and can be fixed for a fee after the event has taken place or for a larger fee [dependent on time] prior to an event. I also agree that any promotion for the event a donated item is used for should always be accompanied with the proper artwork credit to Zephyr Lane Stained Glass either in caption or linked to www.zephyrlaneglass.com.
